SUPPORT COORDINATION

​

Level 2 Support Coordination

This type of support can assist you to understand your NDIS plan and what the different funding categories can be used for.

We can help connect you with providers who are right for you and who will support you in the way you choose. Support Coordinators can work with you to ensure you get the most out of your NDIS funding by providing you with ideas and creative options to support you in working towards your goals.

We can also help you gather reports from your services and ensure you are well prepared when it is time for your NDIS plan to be reassessed. This process can be overwhelming, and we can support you with this as little or as much as you need. Support Coordinators will also link you in with any mainstream supports and services you may want to access. This could include: Health services, Housing support, community-based activities in your local area etc.

Level 3 Support Coordination

This support is designed and funded for participants who have much more complex lifestyle situations than others. These may relate to legal issues, family complexities, significant behavior, mental health implications, or severity of the impact of disability upon your health, capacity, and wellness.

Our Level 3 (Specialist) Support Coordinators have extensive combined experience working with the Complex Support Needs pathway, Queensland Health including hospital discharges after long-term admissions, complex Child Protection/NDIA intersections, working alongside DSDSATSIP and providers where QCAT is involved in approving PBSPs, Youth and Adult Justice involvement, and situations where there are multiple government agencies working together.

Locations

Fika Specialist Services is primarily based in Southeast Queensland, with Support Coordinators providing services in the following areas:

All Brisbane suburbs

Ipswich

Caboolture

Gold Coast

Logan

Toowoomba

Sunshine Coast

Moreton Bay region

We also occasionally accept referrals from outside of these areas if the benefit to the Participant of having our expertise and/or experience outweighs our reduced knowledge of their local area.
